Submit all of the following to receive your rebate: (NOTE: Rebate forms must be postmarked no later December 31, 2008 and received by January 16, 2009.)

• Download and complete the Mail-in Rebate form.
• Clip out original product label with serial number from the bottom of the product package. Do not submit a photocopy of the original.
• Make a photocopy of the original purchase receipt detailing the date and product purchased.
